First and foremost, you must bring in some
picture ID
to help establish your identity.
Secondly, you will need to bring in
some recent doctor's notes that both document your chronic condition
and your ongoing care by a clinician. Records from a doctor of
chiropractic or a medical doctor will be of help. These are required
by the Medical Board. Also, they are needed for us to adequately evaluate your
condition.
We understand that obtaining these
records can be difficult or confusing, and, judging by what we see, you
will probably need help with this process. This is where our front
office can help. Please call and see how this process is done and, if
necessary, have the records sent here first so we can prescreen your
qualifications to avoid problems. There are forms on our website ( see
below) that are designed to help with this issue ( the "how to obtain
medical records" and "medical record's release form").
A good example of the type of records needed is a copy of the doctor's
soap notes (the office will know what this means) preferably notes which
show the severity of the problem and demonstrate how long you've had the
problem and how the diagnosis was reached.
If you cannot get the records, a letter demonstrating the above would
suffice. There need not be any mention of the fact that it is being used
for a cannabis evaluation.
Remember that the above requirements
are specified by the Medical Board and, unfortunately, just the obvious
presence of a disease is not enough to fulfill this requirement. This
visit will be a consultation and as such it must be accompanied by
records for us to review.
The following forms are used in our office and will be needed for an
appointment. You may download these forms, fill them out and bring them
with you for your visit. This will help reduce your wait time.
Remember that we will schedule an actual visit time for you and that we
do not "wave"( schedule many at once) schedule. Being on time is a great
help in keeping on schedule for all others involved.
